Tanzania Embassy Application Forms
Access all application forms needed for Tanzania embassy services. Most forms can now be completed online through the official e-Services portal.
Online Applications: Tanzania has moved many services online. Passport applications can be submitted at eservices.immigration.go.tz and visa applications at visa.immigration.go.tz.

Required Documents for Passport Application
- Completed passport application form
- Original Tanzanian birth certificate
- National ID card (Kitambulisho cha Taifa) or citizenship certificate
- Two passport-size photographs (white background, 45mm x 35mm)
- Previous passport (for renewals)
- Payment receipt for passport fees ($90 abroad)
- Proof of residence in the host country

Since January 2026: All visitors to Tanzania must obtain an e-Visa before arrival. Visa on arrival is no longer available. Apply online at visa.immigration.go.tz.

Tips for Completing Forms
- Use black ink for handwritten forms and write clearly in block letters
- Complete all fields - leave no blanks; write "N/A" where not applicable
- Provide original documents - embassies typically require originals plus photocopies
- Photographs must be recent - taken within the last 6 months, passport-size, white background
- Schedule an appointment - many embassies now require appointments for in-person submission
- Check processing times before your travel date to allow sufficient time
- Keep copies of all submitted forms and documents for your records
